Helpful points. I’ve seen the same pattern in messy orgs: when accountability is low, the system often defaults to gatekeeping and late-stage vetoes instead of clear decisions. Clear expectations, basic training, and a simple decision log can help more than adding another influential person. I see this as a UX/service design problem, and making decisions visible builds trust and follow-through.
